HISTORY
   2004 INFORMATION
Eligibility
   Professionals: Minnesota Section PGA Members and Apprentices and non-PGA professionals who currently reside and have physically resided within the territorial boundaries of the Minnesota Section for a minimum of six (6) months since the completion of the preceding State Open shall be eligible to compete in any qualifying event. Any past champion or current exempt player is eligible to participate regardless of his residency.
   Amateurs: Amateur players must have a USGA handicap of six (6.0) or less when computed in accordance with the USGA specifications. Amateurs meeting the criteria for Section residency outlined above shall be eligible to compete in any qualifying event.
   Note: The Minnesota Section encompasses Minnesota, North and South Dakota; Saint Croix County, Wisconsin; and the area in and around Superior, Wisconsin.

Schedule
   Friday, June 25 through Wednesday, July 14: 18-hole qualifying for non-exempt players.
   Friday and Saturday, July 23-24: First and second rounds. Tee times begin at 7:30 am each day. Following the completion of the second round, the field will be cut to the low sixty (60) and ties.
   Sunday, July 25: Final round. Tee times begin at approximately 8:30 am.

Entry Form
   Application for entry must be made on an official entry form or completed on-line. Facsimiles are acceptable with credit card information. Additional entry forms are available from the Minnesota PGA by calling 763-754-0820. The Minnesota PGA reserves the right to accept or reject any entry. All entry confirmations will be posted at www.minnesotapga.com under the confirmed entries link. No confirmations will be mailed.

Entry Fees
   Players who withdraw from either the qualifying round or the tournament before the entry deadline date will receive full refunds. Players who withdraw after the final closing of entries and at least two business days before the appointed qualifying round will receive a refund less a $25.00 processing fee. No refunds will be issued after Wednesday, July 14, 2004. Refunds will be made following the completion of the tournament proper.

Entry Deadlines    All entry applications must reach the Minnesota PGA office by 4:30 pm on Wednesday, June 16, 2004.
   All players who are exempt from qualifying (see list on page 2) are still responsible for filing their entry applications before the entry deadline. Mail applications early to allow for delay or error in transmission. Deadline for entries means time of receipt at the Minnesota PGA office, NOT time of postmark. Late or incomplete applications are not acceptable.

Rules of Golf
   Play will be governed by the 2004-05 USGA Rules of Golf and Decisions and amended by supplementary rules of play when applicable.

Golf Balls
   Golf Balls must comply with the USGA’s list of conforming golf balls. The one ball condition will be in effect in both the qualifying rounds and the Championship.

Caddies
   Players must make their own caddy arrangements and are responsible for the conduct of their caddies.

Golf Cars
   The use of motorized golf cars will be permitted in qualifying rounds only. Motorized golf cars will not be permitted in the Championship unless a medical condition warrants a waiver of this condition. Waiver requests must be received by Wednesday, July 14, 2004.

   Contact Bunker Hills Golf Club for practice round availability at 763-755-4141. Bunker Hills policies regarding tee time reservations apply.

Personal Appearance
   Players are reminded that proper golfing attire is required. The wearing of T-shirts, shorts, or blue jeans is not allowed during qualifying and Championship rounds.
